Lewis & Clark Sports Medicine

The Sports Medicine Department is charged with helping provide a safe and healthy athletic experience at Lewis & Clark. Student-athletes are required to have a pre-participation physical examination prior to their first and third years of athletic participation at L&C and a medical history update, along with proof of primary health insurance annually. All first-year student-athletes (freshmen and transfers) are required to submit proof of a Sickle Cell Trait blood test. The submission of all information is done through SportsWare, our electronic medical records software.

Below, you will find links to 2 tutorials and 2 checklists (one for first year/transfer student-athletes and one for returning student-athletes) with information about how to set up your SportsWare profile, complete the appropriate sections, and upload the required documents.  You will also find the Lewis & Clark Physical Exam Form (PDF). This exam must be done AFTER June 1, 2025 and submitted on the Lewis & Clark form. Substitute forms will not be accepted. Please take this form with you to your physical exam and have the doctor complete it. Once complete, you will upload and submit this form through SportsWare.
 
In SportsWare you will find a section asking about your medical history.  Please give a complete explanation of any "Yes" answer including dates of injury, illness, and/or surgery. Please have the treating physician complete the Return to Athletics form (PDF) if you have any of the following:
  • A heart murmur or other cardiac abnormality
  • Absence of a paired organ
  • Major illness within the last 12 months
  • Physical injuries requiring ongoing medical attention or clinical rehabilitation in the last 18 months.
  • Any surgery in the past 24 months. A copy of the operation report is also required.
In the appropriate area, please upload a copy (front and back) of your primary (and secondary if applicable) insurance card. This is required. Please review the Lewis & Clark policy on comparable health insurance and make sure your primary insurance meets these criteria before you waive the student health insurance plan.  If you are a Medicaid participant from a state other than Oregon or Washington, please review the information on the school's website.  Here you will find information on how to transition to Oregon's Medicaid plan.
